ahahaha I actually had to look it up after the episode too. Apparently theres an old saying that if you masturbate, you will get hairy palms, as a way to deter kids from it. If you remember at the hospital, Dean was checking out a nurse, and later when he shows Sam his palm, he says, "I was bored, the nurse was hot." and you can guess what he probably did. Sam responds by saying, "you know you could go blind," which was another thing they used to say about those who masturbate.
